All students need to complete the following three steps:

Please complete this form one time for each student in the program.

Step #2:  All students need to have a medical form on file with the boosters and provide an update if there are an changes.

Step #3:  All students must be registered for 'INSTRUMENTAL MUSIC' with the NHS Co-Curriculuar Office before July 18, 2024
